Biotin, Five Carboxylases, and Hair Loss in Deficiency

Biotin is vitamin B7, a water-soluble cofactor. Folli-Activ includes biotin in its oral 10-in-1 formula. The firm statements available from the papers opened for this essay are biochemical and clinical-descriptive. They describe what biotin does in enzymes, what happens to hair when biotin is deficient, and how often low serum biotin showed up in one clinic of women who came in for hair loss. They do not show that a biotin capsule was tested as a stand-alone treatment for androgenetic alopecia in people with normal biotin levels. That claim is omitted because it is not the record these papers contain.

Five enzymes, and an intake figure

Zempleni, Hassan, and Wijeratne review biotin and biotinidase deficiency. Biotin is the essential coenzyme for five mammalian carboxylases: acetyl-CoA carboxylase alpha, acetyl-CoA carboxylase beta, propionyl-CoA carboxylase, 3-methylcrotonyl-CoA carboxylase, and pyruvate carboxylase. Biotin is bound to a specific lysine in each enzyme. The reactions fix bicarbonate. The pathways are fatty acid synthesis, gluconeogenesis, and replenishment of the tricarboxylic acid cycle, along with branched-chain amino acid metabolism. Carboxylase activity falls substantially when biotin is lacking. The adequate intake they cite for adults is 30 micrograms a day.

The same review lists the deficiency picture: seizures, hypotonia, ataxia, dermatitis, hair loss, intellectual disability, ketolactic acidosis, organic aciduria, and fetal malformations. Causes they name include low dietary intake, drug-vitamin interactions, and failure of the proteins that handle biotin: the sodium-dependent multivitamin transporter, monocarboxylate transporter 1, holocarboxylase synthetase, and biotinidase. Hair loss is on the symptom list of the deficiency state. That is a direct statement from a vitamin review, and it is the reason biotin belongs in a conversation about hair.

Biotin is also attached to histones. Zempleni notes that biotinylated histones are enriched in repeat regions of the human genome and participate in transcriptional repression and genome stability. The carboxylase role is the one tied, in the same paper, to the deficiency syndrome that includes hair loss. The histone role is a second biochemical fact, and this page does not stretch it into a hair-count claim.

What a hair-loss clinic measured

Trüeb measured serum biotin in women who complained of hair loss. Biotin deficiency was present in 38 percent of that group. Among the women whose trichograms showed diffuse telogen effluvium, who were 24 percent of the series, 35 percent had a seborrheic-like dermatitis with it. About 11 percent of the women with biotin deficiency had a personal history of a risk factor for deficiency. Trüeb's conclusion is operational: indiscriminate oral biotin for every woman with hair loss is the wrong routine, and biotin supplementation belongs where deficiency, and its relevance to that person's hair loss, has been shown by history, examination, a serum level, and exclusion of other causes.

Patel, Swink, and Castelo-Soccio reviewed case reports and trials of biotin for hair and nails. They found 18 reports. In every case, the person given biotin had an underlying reason for poor hair or nail growth. Those cases improved after biotin. The review states that the supporting clinical material is about acquired and inherited biotin deficiency and about specific disorders such as brittle nail syndrome and uncombable hair. A universal claim for healthy people with normal biotin status is not what that review delivers, so this page does not make that claim.

How to read biotin inside a 10-in-1 bottle

Two facts stand at the same time. Biotin is required for the carboxylases that run fatty acid synthesis and amino acid metabolism, and biotin deficiency includes hair loss. In Trüeb's clinic, 38 percent of women complaining of hair loss had low serum biotin. Separately, patterned androgenetic alopecia has a DHT literature that biotin papers do not replace. Folli-Activ does not ask biotin to do the saw palmetto job or the pumpkin seed oil job. It includes biotin because the vitamin is part of the keratin-and-metabolism layer of the formula, next to ingredients that address other published targets.
